custom model data not working (1.21.3)

Title is self explanatory I believe. I have followed multiple tutorials, but I seemingly can not get it to work. pack is in the attachments - custom model data number is 6080001 I would prefer to hop on a call to do this but via chat is fine as well. thank you in advance!
